Becht Engineering has been involved in failure investigations for over 30 years for both fixed and rotating equipment in the refining and petrochemical industry. Not only does our team bring world class equipment expertise, but we also bring a structured RCFA (Root Cause Failure Analysis) approach which allows us to harness our expertise, in partnership with client team members, to very effectively identify the root cause of failures, and most importantly, develop solutions to eliminate future failures.
While historically many different approaches have been utilized in RCFA, at the heart of our Becht Engineering RCFA work process is “Cause Mapping”. The Becht Engineering cause mapping approach allows us to facilitate the investigation team to deliver comprehensive, data driven failure investigations, eliminating biases which can lead the investigation astray. In addition to facilitation of the formal RCFA, Becht Engineering also brings various discipline subject matter experts with many years of industry experience to complement the client’s team, along with world class materials and engineering analysis capability, to effectively collectively identify the root cause of failure and develop robust solutions.
In addition to identifying root cause of failure, RCFA process also uncovers any management system issues that may exist in the client’s facility, or organization. Uncovering these management system issues not only addresses the particular failure being investigated, but it proactively identifies and eliminates other future failures at the site, or even at a corporate level.
